Your First Visit

Our Practice – Reception Area

When you arrive at our practice, you’ll be greeted by our friendly receptionist and a staff member will assist you with filling out a new patient and medical questionnaire.

Be sure you bring the following items with you on the day of your first visit:

  • Identification
  • A list of any medications you may be taking
  • Your dental history
  • A form of payment (please see the Payment tab for the methods we accept)
  • Patient’s insurance information (including plan ID number)

An assistant, dental hygienist or dentist will come for you when it’s time for your appointment. They will introduce themselves and then lead you to the operatory where your service or treatment will take place. There, your dental history will be reviewed. We may also familiarize ourselves with you by asking you questions about:

  • Your family
  • School or employment
  • And any interests or hobbies.

You will then receive a complete and thorough examination of your mouth and associated areas, including an extraoral and intraoral examination, periodontal measurements to gauge the health of your gums, oral cancer screening, dental health assessment, and if needed radiographs, photos, and models.  We will discuss recommended treatment and optional treatment at this time as well as address any concerns you may have.
